Insurance Information
We accept most major health plans to include variety of HMO’s and PPO’s. We ask that you call the customer service number on your insurance card to ensure that the physician treating you is a participating provider with your insurance plan. If your plan offers out of network benefits, we will file your insurance as a courtesy. However, you will be responsible for the amounts not covered by your insurance. Please be prepared to present your insurance cards and a picture identification card at the time of your appointment. This will ensure that your claims are filed properly. Payment is due at the time services are rendered. As an added convenience for our patients’ we have provided a direct link to some of the major insurance companies so that you may obtain co-pay and deductible information before arriving at our office. Please inquire with our office staff if you have any questions or if you do not see your insurance plan listed. You may also obtain this information by calling the customer service number on your insurance card.
Appointments
If you would like to schedule an appointment please call our office at (210) 477-5151. Be sure to state which location you prefer, our Toepperwein, Stone Oak, or Village Drive location.
We assure you that we will do everything we can to exceed your expectations and our physicians will provide you with highest level of patient care.
As a courtesy to our patients, we do have an automated system that will call to remind you of your appointment time the day before your appointment.
Fees & Payments
We make every effort to decrease the cost of your medical
care. Therefore, we request payment arrangements for all office services at
the time they are rendered unless prior arrangements have been made. We accept checks, MasterCard, Visa, and Discover for your convenience. If we are
a participating provider of your insurance company, we will bill them. However,
payment is the patient's responsibility. We will help in any way we can to assist
you in handling claims.
Prescriptions & Renewals
Please request all prescriptions and authorizations for renewals
between 8:00am - 5:00pm, Monday to Friday, when our full records are available.
Renewals requested at other times will be filled only for extreme or emergent
circumstances.
